We earn a fee upon enrolling a caregiver/patient dyad and a success fee when the family caregiver graduates from their 12-week Activation program. We earn a monthly fee to sustain your desired outcomes, after the initial Activation program.
We provide you with regular reports that chart our progress and provide actionable insights about the caregiver/patient dyads and overall population we are engaging.
